LEWISVILLE, Texas - A man accused of driving while intoxicated is facing a felony intoxication assault charge after a multi-vehicle crash in Lewisville left another driver with a broken neck, according to a Lewisville Police Department arrest report.
Manuel Perez Castillo III, of Lewisville, was arrested on August 7, 2026, and charged with Intoxication Assault with a Vehicle Causing Serious Bodily Injury and Resisting Arrest, Search or Transport.
Castillo's bond was set at $30,000, and he remains in custody at the Denton County Jail.
Multiple Vehicles Found at Crash Scene
According to the police report, Lewisville officers were dispatched to Bellaire Boulevard and South Edmonds Lane after multiple callers reported a major crash.
When Officer J. Slaughter arrived, he found two vehicles in the intersection and a third vehicle farther east in the 900 block of Bellaire Boulevard near Village Drive.
After helping extract one driver from a vehicle, the officer was told that another man was standing near his vehicle and was covered in blood.
The officer located Castillo standing next to a Kia SUV. Castillo had visible injuries to his forehead and hands.
According to the report, Castillo told the officer he had been traveling westbound on Bellaire Boulevard when the vehicle in front of him "slammed on their brakes," resulting in the collision.
Officer Reports Signs of Possible Intoxication
While speaking with Castillo, the officer reported immediately noticing that his speech was extremely slurred and his eyes were red and watery. The officer also detected what he described as a slight odor of alcohol coming from Castillo's breath.
Castillo denied drinking alcohol and told the officer he was "just headed home."
Police said Castillo initially claimed he did not have a driver's license. Another officer checked law enforcement databases and found a valid license associated with him. When questioned about the discrepancy, Castillo denied telling officers that he did not have identification or a license.
Paramedics attempted to evaluate Castillo because of his visible injuries, but according to the report, he refused medical treatment.
The officer continued questioning Castillo and again asked whether he had consumed alcohol. Castillo again denied drinking.
Police said Castillo was slightly swaying while standing and told the officer he was uncomfortable with his balance when asked to move onto a concrete area for standardized field sobriety tests.
He was allowed to sit on a curb while the officer conducted the Horizontal Gaze Nystagmus test.
According to the report, the officer observed all six standardized HGN clues, along with vertical nystagmus. Castillo declined to participate in the remaining field sobriety tests.
Based on the officer's observations and the circumstances of the crash, Castillo was initially placed under arrest for driving while intoxicated.
Police Say Other Driver Suffered Broken Neck
The investigation became more serious after medical personnel at Medical City Lewisville informed investigators that the male driver of the vehicle Castillo allegedly rear-ended had suffered a broken neck.
That injury resulted in Castillo facing a charge of Intoxication Assault with a Vehicle Causing Serious Bodily Injury.
Under Texas law, intoxication assault generally involves operating a motor vehicle while intoxicated and, as a result of that intoxication, causing serious bodily injury to another person.
Police Obtain Blood Search Warrant
According to the report, Castillo refused a requested blood specimen after being taken into custody.
Police subsequently obtained a search warrant for his blood that was signed by Judge Holly Fox.
Castillo was taken to Medical City Lewisville so medical personnel could execute the warrant.
Police allege Castillo refused to cooperate with the blood draw despite being told that officers had obtained a search warrant.
According to the arrest report, Castillo stiffened his body and held his arms tightly against himself while refusing to allow the nurse to perform the blood draw.
A Lewisville police sergeant attempted to reason with Castillo, but officers said he continued resisting.
Officers Say Castillo Kicked Sergeant During Struggle
Police attempted to use a restraint device while executing the blood warrant.
According to the report, Castillo continued tightening his arms against his body and thrashing as officers attempted to restrain him. At one point, police said Castillo kicked a sergeant's hand, causing the sergeant to lose his grip.
Additional officers responded to the hospital to assist.
Castillo was subsequently charged with Resisting Arrest, Search or Transport in connection with his alleged efforts to prevent officers and medical personnel from executing the blood search warrant.
After being released from the hospital, Castillo was transported to the Denton County Detention Center.
